Accounts Payable Requirements

The W9 form is required by the Internal Revenue Service and is required for all domestic suppliers.

The W8 is required for all international suppliers. Please select the appropriate version listed here (https://www.irs.gov/forms-pubs/about-form-w-8)

Non-US Based Companies must also fill out the 1042 Questionnaire

For services performed in California ONLY please fill out the applicable form: California Form 590, or California Form 587

The Electronic Funds Transfer Form , allows Siemens to transmit payment electronically and is required by all Siemens suppliers.

e-Invoicing

With Siemens E-Invoicing, electronic invoicing initiative, you will instantly realize the benefits of a streamlined and automated process. Siemens requires our suppliers to sign up for e-Invoicing.

The SBA Self-Certification Form enables Siemens to accurately report diversity spend to the U.S. Small Business Administration and is required by all domestic suppliers.
