Hospital do Espírito Santo de Évora (HESE) is located in Alentejo, one of the most rural regions of Portugal. It has, on average, the oldest population in the country. Cardiovascular diseases are therfore the leading cause of death in the region.

The benefits of the newly developed Remote Care Management program, however, are demonstrated by Antonio, an 84-year-old man suffering from heart failure. Thanks to the telehealth program, he was able to save a lot of time and effort by avoiding many long-distance trips to the hospital for pre- and post-surgery check-ups.
