Connected cardiovascular care is a concept for cardiology IT that consolidates and streamlines the data flow between imaging, cath lab, reading room, and administration – throughout the cardiology service line. Medical history of patient1: A 76 year old male is deemed as a candidate for transoartic valve replacement (TAVR) by his local heart failure clinic

Clinical pathway:

  1. Comprehensive pre-procedural testing is performed including transthoracic and transesophageal echocardiography, computed tomographic (CT) evaluation of the aortic valve/annulus, aorta and iliofemoral.
  2. The cardiologist reviews the test, leverages interoperability to advanced visualization, and accesses the patient’s prior vascular studies, and confirms that the patient is an appropriate candidate for transfemoral TAVR.
  3. Intra-procedural transesophageal echocardiography with 3D advanced visualization is used to assist valve guidance.
  4. A successful TAVR is performed. Interoperability between the cardiac catheterization lab, ultrasound, hemodynamic system, and cardiovascular imaging and information system enable actionable procedural data to be shared in real-time.
  5. Follow-up: Access to pre and post-procedure transthoracic echocardiography used to confirm valve function.
  6. Administration and controlling: During this patient coronary event all the data points from the clinical, operation and financial aspects have been captured. Interoperability to the financial system ensures timely billing.
  7. Actionable data is also available for analysis to evaluate the outcomes for this patient and the community in which he lives.
